Understanding the Priority: Airway, Breathing, Circulation
The client is exhibiting classic signs of acute
hypocalcemia following a
thyroidectomy, a procedure carrying a significant risk for
hypoparathyroidism due to inadvertent damage or devascularization of the parathyroid glands
[3]. The symptoms of muscle twitching, perioral tingling, and anxiety indicate neuromuscular irritability, a direct consequence of low serum calcium levels. In the NCLEX-RN framework, the nursing process always begins with assessment, but when a client presents with signs of a potentially life-threatening complication, the priority shifts to interventions that address the most immediate physiological risk. Here, severe hypocalcemia can rapidly progress to laryngospasm, seizures, and cardiac arrhythmias, including heart failure as noted in post-thyroidectomy case reports
[1]. The first and most critical action is to stabilize the myocardium and prevent these severe neuromuscular and cardiac events.
Why Immediate IV Calcium is the Priority
The correct intervention is to prepare for immediate intravenous
calcium gluconate administration. This directly addresses the "Circulation" component of the ABC priority framework by stabilizing cardiac cell membrane potentials and preventing lethal arrhythmias. The pathophysiology of
hypoparathyroidism involves a deficiency in
parathyroid hormone (PTH), which normally regulates calcium and phosphate homeostasis
[2]. Without adequate PTH, serum calcium levels drop, leading to a state of increased neuronal excitability. The client's symptoms are not merely discomfort; they are precursors to tetany, convulsions, and cardiac decompensation. A case study on post-thyroidectomy hypocalcemia demonstrated that the resulting cardiomyopathy can be reversible with prompt calcium repletion, underscoring the urgency of restoring physiological calcium levels to prevent organ failure
[1]. Oral supplements act too slowly in this acute, symptomatic presentation and are not the first-line intervention when neuromuscular irritability is already evident.
Analysis of Incorrect Options
While the other options are not harmful, they are not the priority and delay definitive treatment.

Administering oral calcium supplements (Option 1) is an appropriate long-term management strategy for chronic hypoparathyroidism but is insufficient for a client in acute distress. Absorption is slow and unreliable when immediate systemic correction is needed.

Encouraging deep breathing exercises (Option 2) addresses the client's reported anxiety, which is a symptom of the underlying hypocalcemia. Treating the symptom without addressing the cause is ineffective and dangerous, as the client’s condition can deteriorate.

Positioning in semi-Fowler's position (Option 4) is a general comfort and respiratory support measure. While it may be implemented concurrently, it does not take precedence over the pharmacological intervention required to prevent life-threatening progression of hypocalcemia. The nurse must first prepare to administer the IV calcium as prescribed, which is the definitive treatment to reverse the pathophysiological cascade of neuromuscular and cardiac instability [1,2].
