Understanding the Priority: Acute Hypocalcemic Tetany
The client's presentation—muscle spasms, perioral tingling, a positive Chvostek's sign, and a critically low serum calcium level of
7.2 mg/dL—is a classic manifestation of acute hypocalcemia following a thyroidectomy. This occurs because of postoperative hypoparathyroidism, a predominant and severe complication of thyroid surgery where the parathyroid glands are inadvertently damaged, devascularized, or removed, leading to a sudden drop in parathyroid hormone (PTH)
[1]. PTH is essential for maintaining calcium homeostasis; without it, serum calcium levels fall precipitously. The resulting neuromuscular excitability manifests as tetany, which can rapidly progress to laryngospasm, seizures, and life-threatening cardiac arrhythmias if not promptly corrected
[2].
Why Intravenous Calcium is the Priority Intervention
Administering prescribed intravenous calcium gluconate slowly is the correct and immediate priority. In the context of severe, symptomatic hypocalcemia with tetany, oral supplementation is insufficient due to its slow and unreliable absorption. The clinical urgency is to raise the serum ionized calcium level rapidly to stabilize cardiac and neuromuscular cell membranes. Undertreated hypocalcemia can result in cardiac arrhythmias and death, making prompt appropriate treatment the definitive safety intervention
[2]. The directive to administer it "slowly" is a critical safety parameter to prevent hypercalcemia or cardiac irritation during infusion.
Analysis of Incorrect Options

Encourage the client to perform deep breathing exercises: This intervention addresses anxiety or a feeling of chest tightness, which can accompany tetany, but it does nothing to correct the underlying biochemical emergency of severe hypocalcemia. It is a supportive measure, not a priority action for a condition with a high risk of decompensation.

Apply warm compresses to the affected muscles: While this may provide some symptomatic comfort for localized muscle cramping, it is a non-urgent comfort measure. It does not treat the systemic calcium deficit causing the tetany and delays definitive, life-saving pharmacological therapy.

Increase dietary intake of calcium-rich foods: Dietary modification is a long-term management strategy for chronic, mild hypocalcemia, not an intervention for an acute postoperative crisis. A client in a tetanic state is at risk for dysphagia and aspiration, and the gastrointestinal absorption of dietary calcium is far too slow to reverse a critical drop in serum calcium. Postoperative protocols emphasize prompt recognition and treatment with calcium supplementation, not dietary changes, to prevent adverse events
