Understanding the Procedure and Its Risks
Balloon aortic valvuloplasty (BAV) for severe aortic stenosis is a percutaneous cardiac catheterization procedure. During BAV, a catheter with a balloon at its tip is advanced through a major artery—most commonly the femoral artery—to the stenotic aortic valve, where the balloon is inflated to fracture calcified deposits and separate fused commissures, thereby increasing the valve orifice area
[1]. Because the procedure requires arterial access with a relatively large-bore sheath, the integrity of the arterial puncture site is the primary concern in the immediate postprocedure period.
Why Monitoring the Catheter Insertion Site Is the Priority
After any cardiac catheterization, the most common vascular complications are bleeding and hematoma formation at the access site
[2]. A systematic review on post-cardiac catheterization care confirms that hematoma and pain remain frequently reported complications, which can prolong hospitalization and increase costs
[2]. In the context of BAV, the risk is heightened because larger sheaths are used, and patients often receive anticoagulation during the procedure. A developing hematoma can lead to retroperitoneal bleeding, pseudoaneurysm, or arteriovenous fistula if not identified and managed promptly. Therefore, frequent assessment of the insertion site for oozing, swelling, ecchymosis, and a palpable hematoma, along with monitoring distal pulses and limb perfusion, is the most critical nursing action to prevent life-threatening hemorrhage and preserve limb viability.
Analysis of the Other Options
Option 1: Encourage early ambulation to prevent thromboembolism. This is contraindicated in the immediate postprocedure period. After arterial sheath removal, the patient must remain on bed rest with the affected leg kept straight for a prescribed period—typically
4 to 6 hours—to allow adequate hemostasis at the puncture site. Early ambulation would disrupt clot formation at the arteriotomy and precipitate bleeding or hematoma expansion.
Option 2: Administer prescribed anticoagulants as ordered. While anticoagulation may be part of the postprocedure regimen to prevent thrombus formation on the disrupted valve surface or at the access site, its administration is not the most immediate nursing priority. The nurse must first ensure that the access site is stable and that there is no active bleeding before administering any agent that could exacerbate hemorrhage. The assessment for bleeding takes precedence over medication administration in the immediate period.
Option 4: Assess heart sounds for improvement in murmur intensity. Although echocardiographic and auscultatory improvements are expected outcomes of a successful valvuloplasty, this assessment does not address the most time-sensitive safety risk. The murmur of aortic stenosis may decrease in intensity, but restenosis or insufficient gradient reduction is a subacute to chronic concern. In the first hours after the procedure, vascular access complications pose a far greater threat to the patient’s safety than the degree of murmur improvement.
The immediate postprocedure nursing priority following balloon aortic valvuloplasty is vigilant monitoring of the arterial access site for bleeding and hematoma formation, as this directly addresses the most common and potentially serious complications of the intervention
