Understanding Mitral Stenosis in Rheumatic Heart Disease
Rheumatic heart disease (RHD) remains a significant global cause of cardiovascular morbidity, and its classic valvular sequela is
mitral stenosis [1]. When assessing a patient with a history of RHD, understanding the underlying pathophysiology is key to linking the structural damage to the specific auscultatory findings.
In chronic RHD, the inflammatory process leads to fibrosis and calcification of the mitral valve leaflets and subvalvular apparatus. This results in commissural fusion and a narrowed mitral valve orifice, which obstructs blood flow from the left atrium to the left ventricle during
diastole. The turbulent, high-velocity flow across this narrowed valve during ventricular filling generates a characteristic murmur. Because the murmur occurs when the mitral valve is open, it is a
diastolic murmur. The quality is described as low-pitched and rumbling because the pressure gradient is relatively low compared to a stenotic aortic valve. The murmur is localized and heard best at the cardiac apex with the bell of the stethoscope, often in the left lateral decubitus position, which brings the left ventricle closer to the chest wall.
Complications such as left atrial enlargement, atrial fibrillation, and systemic thromboembolism, as highlighted in cases of severe mitral stenosis, further underscore the clinical significance of accurate assessment

Analysis of Auscultatory Options

Option 1: A high-pitched, blowing
systolic murmur at the apex is the hallmark of
mitral regurgitation, not stenosis. In RHD, both lesions can coexist, but this finding describes a different valvular dysfunction

Option 2: A harsh, crescendo-decrescendo
systolic murmur radiating to the neck is characteristic of
aortic stenosis. This murmur is generated by turbulent flow during ventricular systole across a narrowed aortic valve.

Option 3: This is the correct finding. A low-pitched, rumbling
diastolic murmur at the apex is the pathognomonic auscultatory sign of mitral stenosis, directly reflecting the turbulent flow through the narrowed valve during ventricular filling.

Option 4: A high-pitched, blowing
diastolic murmur along the left sternal border is typical of
aortic regurgitation. This murmur is caused by the backflow of blood from the aorta into the left ventricle during diastole.
