Symptomatic bradycardia is a heart rate below 50 beats/min accompanied by hemodynamic instability such as hypotension, altered level of consciousness, or shock. The first-line drug is atropine, given as 0.5 mg IV and repeated every 3–5 minutes as needed.

Atropine blocks vagal tone, increasing the spontaneous firing rate of the SA node and enhancing AV conduction. This raises the heart rate and cardiac output, which in turn increases blood pressure. The maximum total dose is 3 mg; if there is no response, prepare immediately for transcutaneous pacing.

CautionBefore giving atropine, always correct hypoxia and hypovolemia first. Use caution in acute myocardial infarction, as atropine can worsen myocardial ischemia, and be aware that atropine alone may have limited effect in bradycardia with hyperkalemia (BRASH syndrome).
