Two requirements: time and understanding
Under the Mental Health Act (Republic Act 11036), service users admitted to a mental health facility must be informed of their rights within 24 hours of admission, in a form and language they understand. Both conditions must be met; meeting one is not enough. Client 2 was informed 19 hours after admission, in his own language, and restated his rights correctly, so his record shows the rule was met. Each of the other records fails either the time limit or the form-and-language requirement.

Working out the times
Client 1 was admitted at 22:00 Monday and informed at 08:00 Tuesday, 10 hours later, but in English, which he does not read or speak. Client 2 was admitted at 23:00 Monday and informed at 18:00 Tuesday, 19 hours later. Client 3 was admitted at 20:00 Monday and informed at 09:00 Wednesday, 37 hours later, beyond the 24-hour limit. Client 4 was admitted at 21:00 Monday and given printed rights at 07:00 Tuesday, 10 hours later, but he cannot read, so the form was not one he could understand.
Why understanding is the point
The purpose of informing service users of their rights is to allow them to exercise those rights, such as the right to informed consent, to confidentiality, to communicate with others, and to legal representation. Information given in a language or form the person cannot understand does not achieve this. Teach-back, in which the service user restates the rights, is good evidence that understanding occurred, as in Clients 2 and 3. Documentation should record the time, the language and form used, and the person's response.
Watch out! Audit questions like this test both conditions at once. Calculate the hours carefully across midnight, then check the language and form. A record that fails either condition fails the rule.
