Weigh the patient each morning after voiding, before breakfast, on the same scale, and in similar clothing. A change of 1 kg reflects approximately 1 L of fluid gain or loss.
For a patient receiving IV fluids at 125 mL/h, daily intake can reach 3 L, making early detection of fluid overload a safety priority. Daily weight is more objective than fluid-balance charting alone.
Do not weigh on whichever scale is free; inter-device calibration differences can mask small fluid shifts. Evening weights after meals include food and fluid intake, confounding the reading.
