The steps of community diagnosis
A community diagnosis turns raw information about a community into a clear statement of its health problems. The remaining steps in this item must follow a fixed order: collect the data with the residents, organize and present the data in tables and graphs, analyze the data against standards, targets, and past years, and write the community nursing diagnoses. The correct sequence is 4, 3, 1, 2. Each step works on the product of the step before it, so none can be moved earlier.
What each step involves
Data collection gathers demographic, socioeconomic, environmental, health status, and health resource data through surveys, interviews, observation, records review, and the spot map, ideally with residents taking part. Organization and presentation sorts the data into categories and displays them in tables, charts, and graphs so that patterns become visible. Analysis compares the figures with national standards and targets, with past years, and with neighboring areas to find gaps, trends, and their likely causes. Finally, the community nursing diagnoses state the problems identified, which become the basis for setting priorities and planning.
| Order | Step | Output |
|---|
| 1st | Collect the data with residents | Raw data |
| 2nd | Organize and present in tables and graphs | Summarized, visible data |
| 3rd | Analyze against standards, targets, past years | Identified gaps and trends |
| 4th | Write the community nursing diagnoses | Problem statements for planning |
Where the wrong sequences break
Organizing data before collecting them is impossible because there is nothing yet to organize. Analyzing before organizing means comparing scattered raw figures with standards, which risks errors and missed patterns. Writing diagnoses before analysis produces problem statements that have no basis in compared data. Watch out! Two options start correctly with data collection; the difference lies in whether organization comes before analysis and analysis before diagnosis.
Exam takeaway
Collect, organize and present, analyze, then diagnose. Because this barangay has never been assessed, the RHU will use these results as baseline data for its multi-year plan, so involving residents in the process also builds their ownership of the problems and the solutions. After diagnosis, the next steps are prioritizing problems, planning with the community, implementing, and evaluating.
