Understanding the Clinical Scenario
The patient has been on a consistent, long-term regimen of
lorazepam (Ativan), a
benzodiazepine, for six months. The key concept here is
physical dependence, which is an expected neurobiological adaptation that occurs with regular benzodiazepine use, distinct from addiction. After half a year of daily dosing, the patient's central nervous system has become physiologically accustomed to the drug's presence to maintain homeostasis. The most important nursing action is to ensure patient safety by preventing a severe withdrawal syndrome, which can be life-threatening.
Why Immediate Discontinuation is Dangerous
Advising the patient to stop the medication immediately (Option 1) or to switch to an over-the-counter sleep aid (Option 3) is contraindicated and hazardous. Abrupt cessation of a benzodiazepine after prolonged use can precipitate a severe withdrawal syndrome. The clinical practice guideline on benzodiazepine tapering explicitly notes that discontinuation can be difficult and is often unsuccessful because withdrawal symptoms can be severe and long-lasting, especially after long-term use

The Rationale for a Gradual Tapering Schedule
Collaborating with the healthcare provider to develop a gradual tapering schedule (Option 2) is the evidence-based standard of care. The goal of tapering is to slowly reverse the neuroadaptations that have occurred, allowing the patient's own neurotransmitter systems to regain equilibrium. The multi-disciplinary expert panel recommendations emphasize that standard tapering protocols may fail or result in harm from improper utilization of pharmacologic principles

Why PRN Dosing is Inappropriate at This Stage
Suggesting the patient take the medication only when feeling anxious (Option 4) undermines the physiological basis of tapering. A PRN (as needed) schedule creates inconsistent blood levels, which can lead to interdose withdrawal and kindling, a process where each withdrawal episode becomes progressively more severe. Successful deprescribing requires a predictable, steady reduction in receptor occupancy, which is only achievable with a scheduled, gradual taper. The D-PRESCRIBE intervention, a pharmacist-led deprescribing model, reinforces this by using structured communication tools and educational materials to guide a systematic, planned reduction, not an erratic, symptom-driven dosing pattern .
