The patient is receiving
vincristine, a vinca alkaloid chemotherapeutic agent. The most critical dose-limiting toxicity of this drug is neurotoxicity, specifically
vincristine-induced peripheral neuropathy (VIPN). When prioritizing care using the nursing process, the nurse must recognize that sensory and motor neuropathies can lead to irreversible nerve damage, safety risks from falls, and loss of fine motor function if not identified and reported early.
Analysis of the Correct Answer (Option 3)
Numbness and tingling in fingers and toes is the classic clinical presentation of peripheral sensory neuropathy. This finding requires immediate intervention because it signals active
neuroaxonal injury. Research indicates that vincristine disrupts microtubule assembly in axons, leading to the release of structural proteins. Specifically,
neurofilament light chain (NfL), an axonal cytoskeletal protein, is released into the blood and cerebrospinal fluid during this damage [1,3]. A longitudinal cohort study of adults with lymphoma confirmed that elevated plasma NfL levels correlate with patient-reported symptoms of CIPN, validating that subjective reports of tingling represent measurable biological nerve injury
[3]. Early detection is imperative to prevent progression to motor weakness, loss of deep tendon reflexes, or foot drop, which can significantly impair quality of life and motor development in survivors [2,4].

Option 1: Mild nausea and decreased appetite

Option 2: Alopecia and fatigue
Alopecia and fatigue are common, distressing, but non-life-threatening side effects of many chemotherapeutic agents. They do not reflect the specific neurotoxic mechanism of vincristine that leads to functional impairment and require no immediate medical intervention beyond psychosocial support and energy conservation strategies.
Option 4: Temporary constipation
Constipation is an expected anticholinergic-like effect of vincristine caused by its impact on the autonomic nerves supplying the gastrointestinal tract. It is typically managed prophylactically with a bowel regimen. While severe ileus would be a concern, "temporary constipation" is a lower priority than new-onset sensory neuropathy.
Clinical Reasoning and Biomarker Connection
The urgency of reporting paresthesia is underscored by emerging evidence on biomarkers. An in vitro study using induced pluripotent stem cells differentiated into sensory neurons demonstrated that vincristine exposure directly causes axonal degeneration, measurable by NfL release
[1]. Furthermore, while VIPN is often assumed to be symmetric, electrophysiological studies using bilateral nerve conduction studies (NCS) have revealed significant side-to-side differences in nerve function in children exposed to vincristine
[2]. This asymmetry can lead to balance disturbances and altered motor development, making early symptom reporting even more critical. Validated tools like the Pediatric Chemotherapy-Induced Neuropathy (P-CIN) patient-reported outcome measure are being adapted to help clinicians identify these symptoms early and implement interventions before the damage becomes functionally limiting
