Clinical Priority Setting in Severe Anorexia Nervosa
The client presents with a constellation of findings indicative of severe malnutrition secondary to anorexia nervosa: a critically low BMI of
14.5 kg/m², bradycardia (
HR 45 bpm), hypotension (
BP 85/50 mmHg), and hypothermia (
T 96.2°F). In the context of NCLEX-RN prioritization, these vital sign abnormalities are not merely clinical data points; they are the most immediate threats to physiological survival. The principle of airway, breathing, and circulation (ABCs) combined with Maslow’s hierarchy of needs dictates that a hemodynamic and cardiac risk takes precedence over nutritional rehabilitation and psychosocial interventions.
The highest priority intervention is to
initiate continuous cardiac monitoring and frequent vital sign assessment. The profound bradycardia and hypotension signal a severely energy-depleted myocardium with reduced cardiac muscle mass and metabolic rate. This state creates a high-risk substrate for life-threatening arrhythmias. More critically, this client is at imminent risk for
refeeding syndrome once nutritional support begins. The hallmark of this syndrome is a rapid intracellular shift of electrolytes—specifically phosphate, potassium, and magnesium—driven by insulin release following glucose intake in a starved body. The resulting severe hypophosphatemia can precipitate cardiac failure, respiratory muscle weakness, and fatal ventricular arrhythmias
[1]. Therefore, establishing a baseline and maintaining vigilant cardiac surveillance is the foundational safety measure before any caloric intake is introduced.
While implementing a structured meal plan with gradual caloric increases is a core component of medical stabilization, it is a secondary priority. The initiation of nutritional rehabilitation itself is the trigger for the most dangerous complications. The protocol for medical stabilization inherently includes the risk of refeeding syndrome, and outcomes data show that a carefully managed, gradual refeeding process is essential to avoid cardiac collapse
[1]. However, the safety net of cardiac monitoring must be in place first to detect the early electrocardiographic signs of electrolyte derangement, such as QT prolongation or arrhythmias, allowing for rapid intervention.
Psychosocial interventions, including encouraging group therapy and establishing a therapeutic relationship, address the underlying etiology of the disorder. These are crucial for long-term recovery but are not the immediate priority in an acute medical crisis. The client's severe malnutrition can also lead to neurological emergencies, such as
Wernicke encephalopathy, an acute neuropsychiatric condition caused by thiamine deficiency. This can manifest with neurological signs and progress to irreversible brain damage if not promptly recognized and treated . While the client in the scenario does not yet display these neurological signs, the extremely malnourished state necessitates a focus on physiological stabilization and monitoring over psychological exploration. Pharmacological interventions for somatic and psychiatric complications are considered only within a broader medical stabilization plan, which first requires ensuring the client’s cardiovascular system can withstand the metabolic stress of treatment .
