Understanding Subinvolution
After a vaginal delivery, the uterus undergoes a process called
involution, where it rapidly returns to its pre-pregnancy size and condition. This is a critical physiological process to prevent hemorrhage.
Subinvolution is the failure of the uterus to return to its normal size and consistency within the expected timeframe, representing a common puerperal complication
[2]. It is a slower-than-normal or arrested process of uterine muscle contraction and regeneration.
Analyzing the Expected Postpartum Timeline
To identify a deviation, you must first know the normal progression of uterine involution. Immediately after delivery, the uterine fundus is firm and palpable at or slightly below the umbilicus. Over the subsequent days, it descends approximately one fingerbreadth (1 cm) per day into the pelvis. By
10 to 14 days postpartum, the uterus should no longer be palpable abdominally; it has descended completely into the pelvic cavity. A fundus that remains palpable at the umbilicus at
2 weeks postpartum is a classic and strong clinical indicator of subinvolution.

4 to day 10 postpartum. Its presence at
2 weeks can be a normal variant, especially if it is transitioning to lochia alba. While a sudden change or return to bright red bleeding could be concerning, the described finding itself is not the most definitive sign of subinvolution.

Option 3: Episiotomy site with slight redness but no discharge. Slight redness at an episiotomy site without purulent discharge or significant pain is a normal sign of the inflammatory phase of wound healing. It does not reflect the status of uterine muscle contraction or involution.

Option 4: Fundal height palpable at the level of the umbilicus. This is the hallmark sign of subinvolution. The expected descent pattern is disrupted, indicating that the uterine muscle fibers have not effectively contracted and the uterus remains enlarged. uterine fundal height decline as a primary measure of involution, linking a lack of descent to complications . The failure of this process can be associated with conditions like
retained products of conception (RPOC) or
