Any manipulation of the cervix can disrupt the low-lying placenta, leading to catastrophic bleeding. The fetal heart rate (FHR) of
140 bpm with minimal variability is a concerning sign that may indicate fetal compromise due to maternal blood loss and reduced placental perfusion, making immediate and correct action critical.
Analysis of Options
1.
Perform a sterile vaginal examination to assess cervical dilation: This is strictly contraindicated. A digital or speculum examination can puncture the placenta overlying the cervix, precipitating a massive, life-threatening hemorrhage for both the client and the fetus.

Encourage ambulation to promote circulation and reduce anxiety: This is incorrect. Activity and gravity can increase pressure on the placenta and provoke or worsen bleeding. The client requires immediate activity restriction.
3.
Maintain strict bed rest with bathroom privileges only: This is the correct initial nursing intervention. The goal is to minimize uterine irritability and pressure on the cervix to reduce active bleeding. While strict bed rest is often prescribed, bathroom privileges may be allowed if the client is hemodynamically stable and the bleeding has subsided, as a way to balance the risk of thromboembolism. However, the core principle is immediate and significant activity restriction.

Administer oxytocin to control bleeding and strengthen contractions: This is contraindicated. Oxytocin stimulates uterine contractions, which can cause further separation of the placenta from the uterine wall and dramatically worsen the hemorrhage. Tocolytics, not uterotonics, are sometimes used to stop contractions in preterm clients with placenta previa.

Every intervention for a client with placenta previa is aimed at preventing a hemorrhagic event that would force a preterm delivery. The study highlights that the occurrence of APH is the key driver of poor outcomes, not just the diagnosis of previa itself
[1]. Therefore, the nurse's priority is to implement measures that directly reduce the risk of initiating or worsening a hemorrhage. Activity restriction, though its evidence base is debated, remains a foundational nursing measure to minimize mechanical stressors on the lower uterine segment.
The rationale for avoiding a vaginal examination is absolute in known placenta previa. Simulation-based education for obstetrical emergencies, including third-trimester bleeding, is designed to build competency in this exact clinical decision-making [3,4]. A study on an obstetrical emergency simulation module demonstrated that targeted training significantly improves students' competency in managing complex, high-stakes scenarios . This training reinforces the algorithmic approach where "diagnosis and management of third-trimester bleeding" begins with stabilizing the mother and avoiding any exam that could provoke hemorrhage . The consistent emphasis in these educational interventions is on recognizing the clinical presentation and immediately initiating the correct, non-invasive management sequence, which starts with activity restriction and continuous maternal-fetal monitoring, not a pelvic exam.
