Understanding the Clinical Scenario
A primigravida at
36 weeks gestation is presenting for a routine visit. The question asks you to identify the finding that signals a life-threatening emergency requiring immediate intervention. In the context of late pregnancy, you must always be vigilant for the progression of hypertensive disorders of pregnancy to their most severe forms: severe preeclampsia, HELLP syndrome, and eclampsia.
Analysis of the Correct Answer (Option 4)
The sudden onset of severe epigastric pain with visual disturbances is the most concerning finding. This combination represents classic prodromal symptoms of impending eclampsia or severe preeclampsia with underlying hepatic involvement.

Epigastric pain in this context is not heartburn. It reflects hepatic capsular stretching, ischemia, or subcapsular hematoma formation as liver enzymes become elevated and the liver swells. This is a hallmark of severe preeclampsia and a direct warning sign for
HELLP syndrome (Hemolysis, Elevated Liver enzymes, Low Platelets) [1,3].

Visual disturbances (such as scotomata, blurred vision, or diplopia) indicate central nervous system irritability and cerebral vasospasm, which can precede a generalized tonic-clonic seizure (
eclampsia) [2,4].
A recent case-control study identified that while prodromal symptoms like headache, visual disturbance, or epigastric pain have historically shown modest associations with eclampsia, their sudden and severe presentation in combination demands immediate action to prevent maternal seizure and cerebrovascular accident
[2]. Furthermore, a case report on postpartum HELLP syndrome demonstrated that neurological manifestations, including visual disturbances and headache, can precede the full biochemical deterioration of HELLP syndrome, reinforcing that these symptoms are a critical clinical alert even before lab values are grossly abnormal
[4]. Immediate evaluation includes assessing blood pressure, deep tendon reflexes, and laboratory studies for hemolysis, liver function, and platelet count, while preparing to administer magnesium sulfate for seizure prophylaxis.
Analysis of Incorrect Options
Option 1: Blood pressure of 128/82 mmHg with trace proteinuria
While a blood pressure of
128/82 mmHg is in the elevated range for a pregnant individual and trace proteinuria warrants monitoring, this finding alone does not meet the diagnostic threshold for a hypertensive emergency. Severe preeclampsia is characterized by systolic BP ≥
160 mmHg or diastolic BP ≥
110 mmHg, often with significant proteinuria. This finding requires close follow-up but not the same level of immediate, emergency evaluation as neurological or hepatic symptoms.
Option 2: Fundal height measuring 30 cm at 32 weeks gestation
This finding represents a size-dates discrepancy. At
32 weeks, the fundal height in centimeters should approximately equal the weeks of gestation (±
2 cm). A measurement of
30 cm is at the lower limit of normal and could indicate possible intrauterine growth restriction (IUGR), oligohydramnios, or inaccurate dating. While this requires further investigation with ultrasound, it is not an immediate life-threatening emergency for the mother.
Option 3: Fetal heart rate of 155 beats per minute with good variability
A fetal heart rate of
155 bpm is within the normal baseline range (
110-160 bpm). The presence of good variability is a reassuring sign indicating a well-oxygenated fetal central nervous system. This is a normal, expected finding at
36 weeks gestation and requires no further evaluation.
Key Pathophysiological Link
The connection between the symptoms in option 4 and adverse outcomes is well-documented. In a study of severe preeclampsia, progression to HELLP syndrome was associated with significant maternal morbidity, driven by the triad of microangiopathic hemolytic anemia, hepatic necrosis, and consumptive thrombocytopenia [1,3]. The epigastric pain is the clinical correlate of the hepatic ischemia and capsular distension. Simultaneously, the visual disturbances signal that the same endothelial dysfunction and vasospasm are occurring in the cerebral circulation, placing the patient at high risk for posterior reversible encephalopathy syndrome (PRES) and eclampsia
[4]. The study by Hastie et al. emphasizes that refining our recognition of these prodromal symptoms is crucial for timely administration of magnesium sulfate to halve the risk of eclampsia
[2].
