Understanding the Clinical Context
At
20 weeks gestation, a routine ultrasound provides critical information about fetal well-being and anatomical development. The nurse reviewing these findings must differentiate between expected physiological variations and markers that could indicate fetal compromise requiring urgent intervention.
Analysis of the Correct Answer
The most concerning finding is
absence of fetal movement felt by the mother (quickening). While a primigravida may not perceive quickening until
18–20 weeks or later, a complete absence of reported movement at this gestational age, especially when correlated with an ultrasound examination, raises a red flag. The underlying concern is not simply a maternal perception issue but the potential for fetal neurological depression or compromise. Fetal movement is a direct behavioral output of an intact and functioning central nervous system. 110–160 bpm. This value reflects a mature autonomic nervous system regulation and does not warrant concern.
Option 2: Estimated fetal weight at the 25th percentile for gestational age.
An estimated fetal weight (EFW) at the
25th percentile falls well within the normal range. Clinical concern typically arises when the EFW drops below the
10th percentile, which would indicate a small for gestational age (SGA) fetus. SGA is a significant perinatal vulnerability linked to long-term complications across multiple organ systems, including neurological and craniofacial developmental issues . A weight at the 25th percentile simply represents a constitutionally smaller but healthy fetus and does not require immediate escalation.
Option 4: Biparietal diameter measuring 4.5 cm.
A biparietal diameter (BPD) of
4.5 cm is consistent with a gestational age of approximately
19–20 weeks. This measurement aligns with expected fetal growth parameters and provides reassurance about structural brain development. It is not an abnormal finding that would trigger an urgent evaluation.
