Immediate Actions
- Assess patient immediately for any signs of active bleeding (e.g., epistaxis, gum bleeding, hematuria, melena, ecchymosis, altered mental status).
- Obtain vital signs, including orthostatic blood pressure and heart rate, to assess for hemodynamic instability.
- Notify the prescribing provider of the medication error and current patient status immediately.
- Order STAT INR and aPTT levels to determine the degree of anticoagulation.
Anticipated Interventions Based on INR and Bleeding Status
- INR above therapeutic range, no bleeding: Hold next warfarin dose. Administer oral vitamin K as prescribed. Monitor INR frequently.
- INR significantly elevated (e.g., > 10.0), no bleeding: Hold warfarin. Administer oral vitamin K (2.5–5 mg). Recheck INR in 24 hours.
- Any life-threatening bleeding or supratherapeutic INR with major bleed: Administer 4-factor Prothrombin Complex Concentrate (PCC) and intravenous vitamin K (5–10 mg) per facility protocol. Activate rapid response team if hemodynamically unstable.
Ongoing Monitoring and Safety
- Implement bleeding precautions: use soft-bristle toothbrush, avoid intramuscular injections, pad side rails, and prevent falls.
- Monitor vital signs and neurological status every 1–2 hours for the first 24 hours post-overdose.
- Complete an incident report detailing the error, patient assessment, and interventions taken.
- Conduct a root cause analysis to identify system failures and prevent future medication errors.
