While a complete physical examination and review of diagnostic studies are essential components of care, they are time-consuming and should not delay the gathering of focused, symptom-specific data. The initial priority is to obtain a targeted health history that characterizes the pain and associated symptoms, which directly informs the likelihood of cardiac ischemia and guides immediate interventions.
Pathophysiology & Clinical Correlation
Chest pain in the context of coronary artery disease (CAD) arises from an imbalance between myocardial oxygen supply and demand. The patient's presentation—diaphoresis and anxiety—indicates activation of the sympathetic nervous system, a common compensatory response to reduced cardiac output and pain. A detailed history focusing on the PQRST mnemonic (Provocation/Palliation, Quality, Region/Radiation, Severity, Timing) is critical. For instance, substernal pressure radiating to the left arm that began at rest and has lasted for 2 hours strongly suggests an acute myocardial infarction (MI). This information is more immediately actionable than a general physical exam, which may reveal non-specific findings such as an S3 gallop or crackles only after significant ventricular dysfunction has occurred. Clinical Application & Safety
Prioritizing a focused pain assessment aligns with the nursing process and emergency care standards. The nurse must rule out the worst-case scenario first. A detailed health history can quickly identify high-risk features (e.g., crushing pain, nausea, dyspnea) that necessitate an immediate 12-lead ECG and notification of the rapid response team. Delaying this to perform a head-to-toe assessment or to review laboratory results that may not yet be available (such as troponin levels) constitutes a failure in clinical prioritization.
