In Rocky Mountain Spotted Fever, the priority is to administer doxycycline immediately upon clinical suspicion, even before lab confirmation. Delaying treatment increases mortality risk due to progressive endothelial damage.
Monitor for a therapeutic response, primarily defervescence within 24 to 48 hours. Lack of improvement may indicate a severe, complicated course or an alternative diagnosis.
Doxycycline is the first-line treatment for RMSF regardless of age. The risk of dental staining in children is outweighed by the high mortality of untreated RMSF. Standard precautions are sufficient as RMSF is not contagious person-to-person.
