- Strict Hand Hygiene: All staff, visitors, and the patient must perform thorough hand antisepsis before entering the room and before/after any patient contact. This is the single most critical measure.
- Protective Environment: Place the patient in a private room with HEPA filtration and positive-pressure airflow to minimize exposure to airborne fungal spores (e.g., Aspergillus). Keep the door closed at all times.
- Restricted Visitors and Personnel: Screen all visitors for signs of illness. No live plants or fresh flowers are allowed in the room due to the risk of bacterial and fungal contamination. Staff with any infectious symptoms should not be assigned to the patient.
- Dietary Modifications: Implement a low-microbial diet. This excludes raw or undercooked meats, unpasteurized dairy products, and unwashed fresh fruits and vegetables to reduce the ingestion of potential pathogens.
- Meticulous Skin and Mucous Membrane Care: Perform daily full-body skin assessments for any breaks in integrity. Use only soft-bristled toothbrushes for oral care and avoid rectal procedures (thermometers, suppositories) to prevent mucosal trauma and entry points for bacteria.
- Vigilant Monitoring: Monitor vital signs every 4 hours, with particular attention to even subtle temperature elevations. A single oral temperature of >38.0°C (100.4°F) or sustained >37.8°C (100.0°F) for over an hour is a medical emergency requiring immediate blood cultures, broad-spectrum antibiotics, and notification of the provider.
These interventions are directed at the highest-priority problem: the imminent risk of overwhelming sepsis due to the absence of functional neutrophils. All other care, including psychosocial support and managing fatigue and bleeding risk, is secondary to this protective imperative.
