When assessing a 4-year-old child with gastrointestinal symptoms linked to dairy, it is essential to differentiate between various adverse food reactions. The timing and nature of symptoms provide critical diagnostic clues.
Lactose intolerance is a non-immunologic reaction caused by the down-regulation or deficiency of the enzyme
lactase, which is required to digest lactose, the primary sugar in milk

cow milk protein allergy (CMPA), which is an immune-mediated hypersensitivity to milk proteins

Analyzing the Correct Answer
The correct finding is
abdominal cramping, bloating, and loose stools occurring 1-2 hours after drinking milk.
This presentation is classic for lactose intolerance. When undigested lactose reaches the colon, it is fermented by gut bacteria, producing gas and short-chain fatty acids. This process leads to an osmotic shift of water into the bowel lumen, resulting in the hallmark symptoms:
bloating,
abdominal cramping, and
loose, watery stools. The symptom onset of
