Understanding the Pathophysiology
Congenital adrenal hyperplasia (CAH) most commonly results from
21-hydroxylase deficiency, an autosomal recessive disorder that disrupts the adrenal glands' ability to produce
cortisol and, in many cases,
aldosterone [1]. When the adrenal cortex cannot synthesize these vital hormones, the negative feedback loop to the pituitary gland is lost. This leads to excessive secretion of adrenocorticotropic hormone (ACTH), which overstimulates the adrenal glands and shunts precursor steroids toward the production of androgens. The clinical presentation is therefore a combination of
cortisol deficiency,
aldosterone deficiency (in the salt-wasting form), and
androgen excess.
Why Option 4 is Correct
The finding most indicative of CAH in a newborn is
ambiguous genitalia accompanied by
salt-wasting symptoms. This combination directly reflects the underlying pathophysiology. In a
46, XX neonate, the excess adrenal androgens produced in utero lead to the virilization of external genitalia, resulting in ambiguous genitalia
[1]. Concurrently, a deficiency in aldosterone prevents the kidneys from conserving sodium and excreting potassium, leading to the classic salt-wasting crisis. This crisis is characterized by laboratory findings of
hyponatremia,
hyperkalemia, and
metabolic acidosis, which manifest clinically as lethargy, vomiting, diarrhea, and poor feeding in the first few weeks of life
[1][2]. The salt-wasting form is the most severe and requires immediate recognition to prevent life-threatening adrenal crisis
[2].
Analysis of Incorrect Options

Option 1: Low blood glucose can occur in CAH due to cortisol deficiency, as cortisol plays a role in gluconeogenesis. However, a
slow heart rate (bradycardia) is not a primary indicator. The more critical cardiac manifestation of a salt-wasting crisis is related to hyperkalemia, which can lead to arrhythmias and even sudden cardiac arrest, not necessarily isolated bradycardia .

Option 2: Bluish skin (cyanosis) and
breathing difficulty are cardinal signs of congenital heart defects or severe respiratory conditions. While an infant in adrenal crisis may present in shock with poor perfusion, this option does not capture the defining androgen-excess or specific electrolyte disturbance features of CAH.

Option 3: Yellow skin (jaundice) and an
enlarged liver are classic signs of neonatal hepatitis, biliary atresia, or metabolic liver diseases. These findings are not directly associated with the enzymatic defect in CAH, where the primary pathology is in the adrenal glands, not the liver.
Key Clinical and Diagnostic Considerations
For the NCLEX-RN, it is essential to link the assessment finding of ambiguous genitalia with the risk for a salt-wasting crisis. A nurse must prioritize monitoring for signs of adrenal insufficiency, including
lethargy,
vomiting, and
diarrhea, which can rapidly progress to hypovolemic shock
[2]. The diagnostic challenge increases when the presentation is delayed or when a rarer enzymatic defect, such as
3β-hydroxysteroid dehydrogenase deficiency, is involved, as these infants may appear well until a sudden, severe adrenal crisis occurs . The co-occurrence of other congenital anomalies, such as an isolated cleft palate, has been reported but is extremely rare and does not define the condition
[1]. The primary nursing focus remains on early identification of the characteristic virilization and prompt intervention for the life-threatening electrolyte imbalances of the salt-wasting form.
