Understanding Tetralogy of Fallot and Hypercyanotic Spells
The child's sudden onset of cyanosis and restlessness during feeding is a classic presentation of a hypercyanotic spell, also known as a "Tet spell." To understand the priority action, it is essential to grasp the underlying anatomy of Tetralogy of Fallot (TOF). As described in the literature, TOF is a combination of four cardiac defects: a ventricular septal defect, obstruction of the right ventricular outflow tract (pulmonary stenosis), an overriding aorta, and right ventricular hypertrophy

During a spell, an acute increase in right-to-left shunting of deoxygenated blood across the ventricular septal defect occurs, bypassing the lungs and entering the systemic circulation. Events that increase the heart's demand, such as crying or feeding, can trigger a spasm of the infundibular muscle in the right ventricular outflow tract, worsening the obstruction. The priority is to immediately decrease this right-to-left shunt and promote pulmonary blood flow.
Priority Nursing Action: Knee-Chest Position
The nurse's first action should be to
place the child in a knee-chest position. This is a non-invasive, immediate intervention that directly counteracts the pathophysiology of the spell.

Mechanism of Action: The knee-chest position increases systemic vascular resistance (SVR) by kinking the femoral arteries. This increase in SVR creates a back-pressure that opposes the flow of deoxygenated blood from the right ventricle across the ventricular septal defect into the aorta. Simultaneously, the position decreases venous return from the lower extremities, which can help reduce right ventricular preload. The net effect is a decrease in the right-to-left intracardiac shunt, forcing more blood through the stenotic pulmonary artery to the lungs for oxygenation. This physiological mechanism is a core concept in managing acute hypoxemia in infants with cyanotic heart disease .
Why Other Options Are Not the First Priority

Administer oxygen via nasal cannula: While oxygen is a critical intervention, it is not the first action. The primary problem is not a lack of oxygen in the environment but a mechanical shunting of blood away from the lungs. Administering oxygen without first altering the hemodynamics with positioning will have a minimal effect because the blood is not reaching the pulmonary capillary bed to be oxygenated. Oxygen can be applied immediately after the child is positioned.

A spell is a life-threatening emergency, and delaying direct interventions to make a phone call would allow the hypoxemia to worsen. The nurse should implement the knee-chest position and administer oxygen, and then delegate another team member to notify the physician.

Obtain vital signs and pulse oximetry readings: A hypercyanotic spell is a visually recognizable clinical emergency. Taking the time to obtain a full set of vital signs and an oxygen saturation reading, which will be critically low, delays the therapeutic intervention that can immediately begin to reverse the spell. Assessment is continuous during the intervention, not a separate preceding step.
The initial management of a hypercyanotic spell is a high-acuity event where rapid recognition and a simple, physiologically based maneuver can be lifesaving. The knee-chest position is the foundational first step because it mechanically counteracts the shunt, buying time for other supportive measures like oxygen and calming the child to take effect .
