A ventricular septal defect (VSD) is a congenital opening in the interventricular septum, allowing oxygenated blood to shunt from the higher-pressure left ventricle to the lower-pressure right ventricle. This left-to-right shunt increases pulmonary blood flow, leading to pulmonary overcirculation. Over time, the pulmonary vasculature may undergo remodeling, resulting in increased pulmonary vascular resistance and, if uncorrected, pulmonary arterial hypertension (PAH) . The early manifestations of a significant left-to-right shunt are primarily respiratory, as the engorged pulmonary vessels and interstitial fluid accumulation reduce lung compliance and stimulate juxtacapillary receptors, causing tachypnea.
Analysis of Assessment Findings
In a 2-year-old toddler with an uncorrected VSD, the nurse must differentiate between expected compensatory findings and signs of decompensation, particularly the onset of pulmonary edema or worsening heart failure.
- Heart rate of 150 beats per minute: This rate falls within the upper limit of the normal range for a 2-year-old (awake: 80–150 bpm). While tachycardia can be a sign of heart failure, this value alone, in the absence of other signs of decompensation, represents a compensatory mechanism to maintain cardiac output and is less immediately alarming than significant respiratory distress.
- Respiratory rate of 45 breaths per minute: This is an abnormal finding. The normal respiratory rate for a 2-year-old is approximately 22–34 breaths per minute. A rate of 45 breaths per minute indicates significant tachypnea. In the context of a VSD, this finding is the most concerning because it directly reflects the physiological consequence of excessive pulmonary blood flow. The increased left-to-right shunt leads to pulmonary venous congestion and interstitial edema, which are the hallmark precursors to acute pulmonary edema and worsening PAH . This finding requires immediate intervention to reduce the work of breathing and assess for impending respiratory failure.
- Weight gain of 4 ounces in the past month: A weight gain of 4 ounces (approximately 113 grams) per month in a toddler is a classic sign of failure to thrive. Expected weight gain at this age is roughly 4–6 ounces per week. This finding indicates a chronic, ongoing problem related to increased metabolic demand and poor feeding secondary to tachypnea and fatigue. While concerning and requiring follow-up, it does not represent an acute, life-threatening change requiring immediate intervention in the same way that severe tachypnea does.
- Presence of a new systolic murmur: A VSD typically produces a holosystolic or pansystolic murmur heard best at the left lower sternal border. The detection of a murmur is an expected physical examination finding that confirms the diagnosis. A change in murmur characteristics can be clinically significant; however, the mere presence of a murmur is not an acute emergency. In fact, a diminishing murmur in a known VSD can be a late and ominous sign of shunt reversal (Eisenmenger syndrome) as pulmonary pressures equalize, but this is a chronic progression, not an acute finding like severe tachypnea.

This question tests the ability to prioritize nursing assessments using a physiological framework (airway, breathing, circulation). The respiratory rate is the most critical vital sign in this scenario because it directly measures the primary pathophysiological process—pulmonary overcirculation. When analyzing options, an abnormal respiratory rate, particularly one that signals impending respiratory distress, takes priority over a borderline heart rate, a chronic growth issue, or an expected physical finding. The nurse must recognize that tachypnea in a child with a left-to-right shunt is the earliest and most sensitive indicator of worsening pulmonary congestion and heart failure, a concept central to the management of congenital heart disease and its potential complication of PAH .
