Understanding Tetralogy of Fallot and Hypercyanotic Spells
The scenario describes a classic hypercyanotic spell, often called a "Tet spell." In Tetralogy of Fallot (ToF), the primary anatomical defects include a large ventricular septal defect (VSD) and right ventricular outflow tract (RVOT) obstruction. During a spell, the infundibular muscle in the RVOT spasms, acutely increasing the obstruction to pulmonary blood flow. This forces more deoxygenated blood from the right ventricle across the VSD into the systemic circulation, causing a sudden and profound drop in systemic oxygen saturation, leading to cyanosis and irritability

Analysis of the Immediate Nursing Intervention

Option 1: Administer oxygen via nasal cannula at 2 L/min. While supplemental oxygen is often applied, it has limited effectiveness in reversing the core pathophysiology of a Tet spell. The primary problem is not a pulmonary diffusion issue but a mechanical obstruction to blood flow reaching the lungs. Oxygen may slightly reduce pulmonary vascular resistance, but it does not directly relieve the infundibular spasm and can be distressing to a toddler, potentially worsening the spell.

Option 2: Place the infant in a supine position with legs extended. This position is contraindicated. A supine position can decrease systemic vascular resistance (SVR), which increases the pressure gradient favoring a right-to-left shunt across the VSD, thereby worsening cyanosis.

Option 3: Offer oral fluids to prevent dehydration. While dehydration can predispose a child to spells by hemoconcentrating the blood and increasing viscosity, offering oral fluids during an active spell is dangerous. The toddler is irritable and cyanotic, placing them at high risk for aspiration. This intervention is part of long-term management and prevention, not the immediate response to an acute crisis.

Option 4: Position the infant in knee-chest position. This is the correct and most appropriate immediate nursing intervention. The knee-chest position works through a specific physiological mechanism. By flexing the knees to the chest, the nurse increases systemic vascular resistance (SVR) by compressing the femoral arteries. Simultaneously, the squatting-like posture reduces venous return from the lower extremities. The net effect is a rise in pressure in the left side of the heart and the aorta. This increased left-sided pressure opposes the right-to-left shunt across the VSD, forcing more blood from the right ventricle through the pulmonary artery and into the lungs for oxygenation.
