Familial Hypercholesterolemia (FH), a genetic disorder that causes profoundly elevated
low-density lipoprotein cholesterol (LDL-C) from birth
[2]. The underlying pathophysiology involves a mutation in genes critical for LDL receptor function (such as
LDLR,
APOB, or
PCSK9), leading to a failure in clearing LDL-C from the bloodstream
[2]. This results in the deposition of cholesterol in peripheral tissues, forming xanthomas, and within arterial walls, accelerating
premature atherosclerotic cardiovascular disease (ASCVD) [2]. The presence of cutaneous xanthomas is a physical marker of this systemic disease process and a strong predictor of early-onset coronary artery disease, as evidenced by case reports where patients with widespread xanthomas presented with myocardial infarction in adolescence

Complaints of occasional fatigue after physical activity: This is a highly non-specific symptom that could be attributed to numerous causes, including deconditioning, poor sleep, or stress. While fatigue can be an anginal equivalent, particularly in women, it is far less specific and immediately alarming than the objective, visible finding of xanthomas, which serve as a direct biomarker for a high lifetime cholesterol burden

Clinical Application for the School Nurse
For the NCLEX-RN, recognizing that xanthomas are a high-priority, "red flag" assessment finding is critical. In a patient with a family history of cardiovascular disease, discovering tendinous or tuberous xanthomas (commonly on the Achilles tendon, elbows, and knees) should prompt immediate action

541 mg/dL [1]. The nurse's priority is not to diagnose FH but to recognize the finding, document it accurately, and ensure urgent referral for a lipid panel and further cardiovascular evaluation to prevent catastrophic, premature events like myocardial infarction
