The hallmark of sensorineural hearing loss is poor speech discrimination. Clients struggle to distinguish words like "bat" and "pat" even when the volume is adequate, due to degraded neural signal clarity from cochlear or nerve damage.
In contrast, conductive hearing loss typically presents with muffled sounds that become clear when volume is increased. The cochlea is intact, so clarity is preserved once sound overcomes the transmission blockage.
For the Weber test, sound lateralizes to the better ear in sensorineural loss. Lateralization to the affected ear indicates a conductive loss. For the Rinne test, sensorineural loss shows AC > BC (both reduced), while conductive loss shows BC > AC.
Simply amplifying sound for a client with sensorineural loss often makes it louder but not clearer. Assess functional hearing by evaluating speech discrimination, not just pure-tone thresholds.
