For a metallic foreign body in the eye, the immediate priority is to prevent secondary injury from consensual eye movement. Do not irrigate, apply pressure, or attempt removal.
Stabilize the object if protruding, then cover both eyes with a rigid eye shield or sterile gauze pads. This stops the conjugate gaze reflex. Keep the client supine with the head of the bed elevated 30-45 degrees.
Never attempt to remove an embedded object. Never irrigate the eye, as fluid can dislodge the fragment and worsen a corneal laceration. Do not patch only the affected eye.
