Understanding Hyphema and the Priority Nursing Intervention
When a client presents with a hyphema, which is an accumulation of blood in the anterior chamber of the eye, the immediate nursing priority is to prevent rebleeding and reduce the risk of vision-threatening complications. The correct intervention is to elevate the head of the bed to
30-45 degrees and maintain strict bed rest.
Pathophysiology and Clinical Rationale
A hyphema most commonly results from blunt trauma that tears the blood vessels of the iris or ciliary body, causing blood to pool in the anterior chamber

3 to 5 days after the initial trauma. A rebleed is often more severe than the original bleed and dramatically increases the risk of two serious complications: elevated
intraocular pressure (IOP) leading to

corneal blood staining, which can cause permanent vision loss

The priority nursing actions are directly aimed at facilitating the settling of blood and preventing rebleeding. Elevating the head of the bed to
30-45 degrees uses gravity to help the red blood cells settle inferiorly, away from the trabecular meshwork. This settling is crucial because it clears the visual axis and, more importantly, unblocks the eye's drainage system, helping to keep the IOP within a safe range.
