clot retention leading to urinary tract obstruction.

In a female patient, this could suggest a bladder neck obstruction or a mass at the bladder outlet, but it is a less common and less acute presentation of bladder cancer compared to gross hematuria.
Clinical Reasoning & Nursing Implications
The nurse's immediate action upon observing painless gross hematuria with clots should be to ensure hemodynamic stability by monitoring vital signs for signs of hypovolemia, given that cases of life-threatening, transfusion-dependent hematuria from urological sources are well-documented . A large-bore intravenous line should be established, and a blood sample should be sent for a complete blood count and coagulation profile. The nurse must also assess for signs of clot retention, such as suprapubic distension and the patient's inability to void. The finding should be reported immediately to the healthcare provider, as interventions like continuous bladder irrigation (CBI) with a three-way catheter may be necessary to prevent clot formation and obstruction, a management strategy successfully used to resolve hematuria in complex cases .
