Clinical Presentation Analysis
A
45-year-old male presenting with severe flank pain radiating to the groin describes a classic ureteral obstruction pattern. When a stone dislodges from the renal pelvis and enters the ureter, it triggers intense, spasmodic contractions of the ureteral smooth muscle as the body attempts to propel the stone downward. This mechanism produces the characteristic
colicky pain—pain that waxes and wanes in waves—which is a hallmark of
urolithiasis [1]. The radiation pattern follows the anatomical path of the ureter, coursing from the costovertebral angle around to the lower abdomen and into the groin or genital area as the stone migrates distally.
Key Diagnostic Finding
The most indicative assessment finding is the combination of
hematuria and
colicky pain. As a calculus travels through the ureter, its irregular, sharp edges scrape against the urothelial lining, causing direct mechanical trauma to the mucosa and submucosal capillaries. This results in microscopic or gross hematuria. The presence of blood in the urine, coupled with the wave-like pain pattern, strongly differentiates renal calculi from other causes of flank discomfort. Why Other Options Are Less Indicative

Bradycardia and hypotension are not primary features of uncomplicated renal calculi. While severe, intractable pain can occasionally trigger a vagal response with transient bradycardia, this is neither consistent nor specific. Hypotension would raise concern for alternative diagnoses such as a ruptured abdominal aortic aneurysm, particularly given the flank pain radiation, and must be ruled out emergently.

Polyuria and polydipsia are cardinal signs of diabetes mellitus and do not reflect the pathophysiology of an obstructing stone. A stone large enough to cause hydronephrosis bilaterally could theoretically impair renal concentrating ability over time, but this is a late complication, not an acute presenting feature.

Fever and chills with dysuria suggest a concurrent urinary tract infection or, more critically, an infected obstructed system such as
pyonephrosis. This is a urological emergency requiring immediate decompression, as the combination of obstruction and infection can rapidly lead to urosepsis. While a stone can be the underlying cause of the obstruction, the presence of fever shifts the clinical priority from stone diagnosis to sepsis management . In uncomplicated symptomatic urolithiasis, patients are typically afebrile, and clinical decisions regarding early versus late surgical intervention are guided by factors such as stone size, location, and pain control, not by signs of systemic infection .
Diagnostic Reasoning in the Emergency Setting
Non-contrast computed tomography (NCCT) remains the gold standard for confirming the diagnosis, precisely localizing the stone, and assessing for secondary signs such as hydronephrosis
[1]. However, the clinical assessment finding of hematuria with colicky flank pain provides the highest pre-test probability and justifies urgent imaging and analgesia. Recognizing this classic dyad allows the nurse to anticipate the diagnosis, prioritize pain management, and prepare the patient for the appropriate diagnostic workup while remaining vigilant for red flags such as fever or hemodynamic instability that would signal a complicated presentation.
