Understanding Hydronephrosis and Its Clinical Presentation
Hydronephrosis refers to the distension and dilation of the renal pelvis and calyces, typically resulting from an obstruction to the free flow of urine from the kidney. This obstruction can occur at any point along the urinary tract, from the ureteropelvic junction to the urethral meatus. The resulting back-pressure of urine can progressively damage the renal parenchyma, leading to impaired kidney function if not promptly identified and relieved. For the NCLEX-RN, recognizing the classic clinical presentation is essential for early nursing assessment and intervention.
A key characteristic of hydronephrosis is
flank pain, which is often described as a dull, aching discomfort in the costovertebral angle (CVA) or flank region. This pain arises from the stretching of the renal capsule as the collecting system becomes distended with urine. The pain pattern is particularly telling: it is typically constant but can be intermittent, and it is frequently exacerbated by movement or direct palpation over the affected kidney. This finding aligns directly with the case reports provided. For instance, a
35-year-old patient with testicular vein syndrome, a rare cause of ureteral compression, presented with a
2-year history of
intermittent left flank pain as the primary symptom, without any urinary complaints

intermittent severe left flank pain [2]. This consistent presentation underscores that flank pain, particularly when it is the dominant symptom and is reproducible with movement or palpation, is a highly indicative assessment finding.
Differentiating Hydronephrosis from Other Urinary Conditions
To correctly identify the most indicative finding, it is crucial to differentiate the symptom profile of hydronephrosis from other common urinary disorders.

Decreased urine output with concentrated urine: This finding is more suggestive of prerenal acute kidney injury (e.g., from dehydration or hypoperfusion) or intrinsic renal failure, where the kidney's filtering capacity is compromised. In hydronephrosis, the obstruction is post-renal. Urine is still produced, but its drainage is blocked. Output may eventually decrease if the obstruction is complete and bilateral, but the hallmark is pain from distension, not a primary reduction in urine production. The case reports highlight presentations without mention of oliguria, with one patient specifically having
no urinary symptoms [1].

Frequent urination with burning sensation: This constellation of symptoms is classic for a lower urinary tract infection (UTI), such as cystitis or urethritis. The dysuria, urgency, and frequency are caused by inflammation of the bladder or urethral mucosa. While a UTI can be a complication of a long-standing obstruction due to urinary stasis, it is not the primary, defining clinical presentation of the hydronephrosis itself.

Blood in urine with fever and chills: This presentation is a red flag for an upper UTI, specifically
pyelonephritis, or potentially an obstructing infected stone (obstructive pyelonephritis). The systemic symptoms of fever and chills indicate an infectious and inflammatory process within the renal parenchyma. While hydronephrosis can be caused by an obstructing stone, and that condition can become secondarily infected, the combination of hematuria and systemic signs of infection points more directly to an infectious or stone-related etiology. The case of ureteral endometriosis, for example, describes a
"silent" progression to severe hydronephrosis with nonspecific symptoms, not an acute febrile illness

Therefore, the finding most directly linked to the mechanical distension of the renal capsule, which is the core pathophysiological event in hydronephrosis, is flank pain that worsens with movement and palpation. This is a direct consequence of the obstruction, as seen in cases ranging from vascular compression
