The hallmark of acute pyelonephritis is costovertebral angle (CVA) tenderness combined with high fever and systemic signs. This reflects inflammation of the renal parenchyma.
In contrast, cystitis presents with localized symptoms: suprapubic tenderness, dysuria, and urgency, typically without fever or flank pain.
A finding of CVA tenderness with a temperature over 38.0°C requires urgent intervention and urine culture to prevent urosepsis. Do not dismiss upper tract infection in the absence of classic urinary symptoms.
