In a client with COPD exacerbation, the sudden onset of paradoxical chest movement and decreased breath sounds is a red flag for tension pneumothorax. This occurs when a ruptured bleb creates a one-way valve, trapping air in the pleural space.
The trapped air collapses the lung and causes a mediastinal shift, compressing the heart and great vessels. This drastically reduces venous return and cardiac output, leading to cardiovascular collapse. Immediate needle decompression is required.
Do not delay intervention for diagnostic tests like a chest X-ray. Tension pneumothorax is a clinical diagnosis. A finding of tracheal deviation away from the affected side is a late and ominous sign. Prioritize this over other signs of respiratory distress like tachypnea or low oxygen saturation.
