An International Normalized Ratio (INR) greater than 1.5 is generally considered a contraindication for elective bronchoscopy with biopsy. A value of 3.5 indicates a severe coagulopathy and an extremely high risk of uncontrolled hemorrhage.
The nurse must immediately hold the procedure and communicate this critical lab value to the healthcare provider. Anticipate orders to administer vitamin K, fresh frozen plasma (FFP), or prothrombin complex concentrate (PCC) to reverse the coagulopathy before the procedure can safely proceed.
Do not proceed with the biopsy. Vital sign variations like mild hypertension or low-grade fever are secondary concerns compared to a critical INR. Document all communication and the time-out process thoroughly.
