When assessing a patient with hyperthermia, the nurse must rapidly differentiate between varying degrees of heat-related illness to prioritize interventions. altered mental status combined with
absent sweating (anhidrosis). This combination signals a catastrophic breakdown of homeostasis. Normally, the body maintains a constant internal temperature through heat dissipation mechanisms, primarily evaporation via sweating
[2]. When a patient stops sweating in the context of a high environmental heat load, the body has lost its primary cooling mechanism, causing the internal temperature to rise uncontrollably, often exceeding
40.5 °C [2]. The concurrent central nervous system dysfunction, manifesting as altered mental status or delirium, is a defining characteristic of heatstroke and indicates that the hyperthermia is causing direct neuronal injury and systemic inflammation

Profuse sweating with extreme fatigue indicates that the body’s cooling mechanisms are still functioning, a hallmark of heat exhaustion. While this requires prompt intervention, it does not represent the same immediate threat to life as thermoregulatory shutdown. Similarly,
nausea with a mild headache and
flushed, warm skin are common findings in heat exhaustion and milder heat-related conditions. Flushed skin results from cutaneous vasodilation, a compensatory attempt to dissipate heat through radiation and convection
[2]. The presence of these signs, without central nervous system dysfunction, suggests the patient’s condition has not yet progressed to the critical stage of heatstroke
