Understanding the Priority: Cushing's Triad
The most concerning finding requiring immediate intervention is
Cushing's triad. To understand why, we must look at the physiological progression of
increased intracranial pressure (ICP). The brain is encased in a rigid skull, and as pressure rises from a mass, edema, or hemorrhage, cerebral perfusion is compromised. The body initiates a powerful compensatory response to maintain blood flow to the brain, and Cushing's triad represents the final, desperate stage of this compensation. It signals that the brainstem is under severe mechanical stress and is about to herniate.
The triad consists of three specific findings:
