Analysis of Other Options

Option 1 (Absence of pedal pulses): This is a
late finding in ACS and a hallmark of direct arterial injury. In ACS, the compartment pressure (typically >30 mmHg) is sufficient to collapse capillaries but is usually lower than systolic blood pressure, meaning a palpable pulse can still be present even with severe muscle ischemia
[1]. While a vascular injury must be ruled out, the absence of a pulse points to a macrovascular problem, whereas the paresthesia and pallor point to the microvascular crisis of ACS, which is more immediately limb-threatening if the arterial injury is not the primary cause.
