Understanding Bone Densitometry (DEXA)
Bone densitometry, commonly known as a DEXA (Dual-Energy X-ray Absorptiometry) scan, is the gold standard for diagnosing osteoporosis. It works by passing two low-dose X-ray beams through the bone, and the amount of energy absorbed by the bone mineral is measured. The result is a
T-score, which compares the client's bone mineral density (BMD) to that of a healthy young adult. A T-score of
-2.5 or lower indicates osteoporosis
[1]. The goal of pre-procedure instructions is to ensure the scan's accuracy by eliminating factors that can artificially alter the BMD reading.
Why Discontinuing Calcium Supplements is the Priority
The correct instruction is to discontinue calcium supplements
24 hours before the test. The rationale is directly tied to the scan's technology and the potential for a significant artifact. Calcium supplements, particularly in tablet form, may not be fully dissolved and absorbed in the gastrointestinal tract before the scan. Undissolved calcium tablets or concentrated calcium residue in the stomach or intestines can appear as a dense, radio-opaque area on the DEXA image. Because the scan measures X-ray absorption, a concentrated bolus of calcium overlying the lumbar spine or hip region can be misinterpreted by the scanner's software as bone mineral, leading to a falsely elevated BMD reading. This artifact masks true bone loss and can lead to a missed diagnosis of osteopenia or osteoporosis. The importance of accurate dietary and supplemental calcium assessment is underscored by research showing that low calcium intake is a critical, and often underrecognized, risk factor for osteoporotic fractures, making it essential to not let supplementation obscure a true deficiency state

Analysis of Incorrect Options
1. Remove all jewelry and metal objects before the scan.
While removing metal objects is a standard instruction for many radiologic procedures like standard X-rays or CT scans, it is not the
most important instruction for a DEXA scan. Metal zippers, snaps on clothing, or jewelry can indeed cause minor artifacts. However, the DEXA scan's low-energy X-ray beams are highly attenuated by metal, creating an obvious signal void (a black area) or streak artifact on the image, not a false increase in density. The technologist can easily identify and exclude this from the analysis. The artifact from an undissolved calcium tablet is more insidious because it mimics bone density, potentially going unnoticed and directly corrupting the diagnostic T-score.
3. Maintain NPO status for 8 hours prior to the procedure.
This instruction is not evidence-based for a standard DEXA scan. The procedure does not require sedation, contrast media, or an assessment of organs affected by digestion. While a very heavy meal could theoretically introduce minor artifacts, it is not a standard requirement. The specific concern is not food itself, but the presence of a concentrated, non-absorbed mineral supplement. This option confuses DEXA preparation with that of procedures requiring anesthesia or gallbladder studies.
4. Perform deep breathing exercises during the scan.
This instruction is contraindicated. For a DEXA scan to be accurate, the client must lie completely still on the padded table while the scanning arm passes overhead. The scan time is short, typically
10 to 20 minutes. Deep breathing exercises would cause movement of the rib cage, spine, and pelvis, resulting in a blurred image that cannot be accurately analyzed for bone density. The correct instruction is to breathe normally and lie still.
Connecting to Osteoporosis Pathophysiology and Risk
The critical nature of this pre-procedure instruction is magnified by the clinical context of osteoporosis risk. The DEXA scan is often ordered for postmenopausal women, a population in whom the duration of menopause is a strong predictor of bone loss . In these clients, missing a diagnosis due to a falsely elevated BMD from calcium supplement artifact has serious consequences, delaying crucial treatment. Furthermore, the interplay between calcium and vitamin D metabolism is central to bone health, and accurate assessment of baseline BMD is needed to guide supplementation therapy, which is a cornerstone of managing several endocrine disorders that affect the skeleton . An instruction to hold calcium supplements for
24 hours is a simple, high-impact step to ensure the scan reflects the client's true bone density, not their recent supplement intake.
