Understanding Cellulitis and the Priority Intervention
Cellulitis is an acute bacterial infection of the dermis and subcutaneous tissue, most commonly caused by
Streptococcus pyogenes or
Staphylococcus aureus. The infection triggers a robust inflammatory response, leading to the classic signs of erythema, localized warmth, swelling, and pain
[1]. The swelling, or edema, is not just a symptom; it is a key driver of ongoing tissue damage. Increased capillary permeability allows fluid and inflammatory cells to leak into the interstitial space, which can impair local blood flow and lymphatic drainage, delaying healing and increasing pain.
The priority nursing intervention for a client with cellulitis of the lower leg is to
elevate the affected extremity above the level of the heart. This action directly counteracts the pathophysiology of the condition. By using gravity to promote venous and lymphatic return, elevation reduces the pooling of interstitial fluid. This decrease in edema relieves pressure on nerve endings, thereby reducing pain, and improves microcirculation to the infected tissue, which is essential for delivering antibiotics and oxygen while removing metabolic waste products. In older adults, this intervention is particularly crucial. Analysis of Incorrect Options

Option 1: Apply cold compresses to reduce inflammation. While cold can reduce inflammation in musculoskeletal injuries, it is contraindicated for cellulitis. Vasoconstriction caused by cold application will further compromise blood flow to an already infected and potentially ischemic area, hindering the body’s ability to fight the infection and deliver systemic antibiotics to the site.

Option 2: Encourage frequent ambulation to prevent blood clots. Ambulation is important for preventing venous stasis and deep vein thrombosis (DVT) in many hospitalized patients. However, in the acute phase of lower leg cellulitis, the affected limb is painful, swollen, and inflamed. Ambulation increases blood flow to the extremity, which can exacerbate edema and pain, and may theoretically promote the systemic spread of bacteria. Rest and elevation are the priorities until the acute inflammation begins to subside.

Option 4: Massage the affected area to improve circulation. Massaging an area of acute cellulitis is strictly contraindicated. The mechanical pressure can disrupt local tissue defenses, damage fragile, inflamed capillaries, and potentially dislodge bacteria into the lymphatic or circulatory system, risking bacteremia and the spread of infection.
