Understanding the Priority: Airway and Breathing in Pulmonary-Renal Syndrome
The client is presenting with Goodpasture's syndrome, a classic cause of pulmonary-renal syndrome characterized by the simultaneous occurrence of
diffuse alveolar hemorrhage (DAH) and rapidly progressive glomerulonephritis. The clinical cues are
hemoptysis (coughing up blood from the lungs) and
hematuria (blood in the urine). While both the pulmonary and renal manifestations require attention, the immediate threat to life comes from the respiratory system.
Pathophysiology and Clinical Reasoning
In Goodpasture's syndrome, autoantibodies attack the basement membrane in both the kidneys and the lungs. The pulmonary manifestation, DAH, is a catastrophic event. As noted in the review by Dutta et al., DAH arises from immune-mediated injury to the pulmonary microcirculation, leading to bleeding into the alveolar spaces
[1]. This process directly impairs gas exchange. The alveoli, which should be filled with air, become filled with blood, preventing oxygen from reaching the bloodstream and causing profound hypoxemia. This can rapidly progress to acute respiratory failure and is associated with a high mortality rate
[1]. The case report by Vaezi and Amini similarly highlights a clinical presentation of hemoptysis and diffuse pulmonary infiltrates, which required advanced support with veno-venous extracorporeal membrane oxygenation (VV-ECMO) for severe respiratory failure
[2].
Therefore, the nurse's priority is to apply the ABC (Airway, Breathing, Circulation) framework. Hemoptysis is a direct signal that the airway and breathing are compromised. Monitoring
oxygen saturation provides a direct, real-time measurement of the severity of this gas-exchange impairment. Maintaining a patent airway is the foundational intervention to ensure that whatever functional lung tissue remains can be ventilated. A drop in
SpO2 or signs of increased work of breathing would necessitate immediate escalation, including supplemental oxygen and notification of the provider for potential emergency airway management.
Analysis of Other Options

Option 1 (Encourage increased fluid intake): While maintaining hydration is a supportive measure for many conditions, it is not the priority in an acute DAH scenario. Aggressive fluid administration could theoretically worsen pulmonary edema and gas exchange in the setting of alveolar capillary injury. The immediate life threat is hypoxia, not dehydration.

Option 2 (Position in Trendelenburg): The Trendelenburg position (head lower than feet) is contraindicated in a client with active hemoptysis and respiratory distress. This position would place the client flat or head-down, increasing the work of breathing and the risk of aspiration of blood. The priority position would be upright, such as high-Fowler's, to optimize lung expansion and facilitate expectoration.

Option 4 (Administer iron supplements): The client is at risk for anemia due to both chronic renal disease and acute blood loss into the lungs. However, iron supplementation is a long-term management strategy to address a secondary consequence of the disease. It does not address the immediate, life-threatening problem of impaired oxygenation from active alveolar bleeding. The priority must be to stabilize the client's respiratory status first.
