Clinical Context and Priority Assessment
This client is presenting with a classic
vaso-occlusive crisis (VOC), the hallmark of sickle cell disease (SCD) and the most common reason for emergency department visits
[3]. While pain is the primary symptom, the critical nursing responsibility is identifying the early transition from an uncomplicated VOC to a life-threatening complication:
acute chest syndrome (ACS). ACS is defined as a new pulmonary infiltrate accompanied by fever and/or respiratory symptoms, and it is a leading cause of death in adults with SCD

Analysis of the Correct Answer (Option 3)
A respiratory rate of
32 breaths per minute with shallow breathing is the most concerning finding. This assessment directly points to the potential development of ACS. The underlying pathophysiology involves the sickling of red blood cells within the pulmonary microvasculature, leading to regional hypoxia, inflammation, and ventilation-perfusion mismatch. The rapid, shallow breathing is a compensatory mechanism for the sudden drop in lung compliance and gas exchange impairment. More critically, the "shallow" component indicates a high risk for progressive atelectasis and hypoventilation, which further exacerbates sickling by promoting regional hypoxia. This finding demands immediate intervention, specifically the initiation of
incentive spirometry, which is a cornerstone of care during a VOC to prevent ACS
[1]. Without prompt nursing action to improve lung expansion, this client is on a trajectory toward respiratory failure.
Why the Other Options Are Less Immediately Critical
Option 1: Hemoglobin level of 7.2 g/dL with no active bleeding.
A hemoglobin of
7.2 g/dL represents chronic, compensated anemia typical of SCD. In the absence of active bleeding or hemodynamic instability, this baseline anemia does not require an emergent intervention like transfusion. Transfusion therapy in SCD is reserved for specific indications such as acute symptomatic anemia, stroke, or severe ACS, not a stable low hemoglobin level
[1]. The body has adapted to this chronic state, and the immediate threat to the airway and breathing takes priority.
Option 2: Oxygen saturation of 92% on room air at rest.
An oxygen saturation of
92% is below the typical normal range but is a common finding in SCD and does not automatically signal a crisis. Guidelines for managing VOC emphasize targeting a specific oxygen saturation level, and routine supplemental oxygen is not recommended unless hypoxemia is clearly documented
[1]. The primary concern is not the number itself but the underlying cause. The tachypnea and shallow breathing are the clinical signs that indicate the respiratory system is failing, which may or may not yet be reflected in a critically low SpO2. Treating a number with oxygen without addressing the ineffective breathing pattern is a missed priority.
Option 4: Temperature of 100.8°F (38.2°C) with clear lungs.
A low-grade fever of
100.8°F (38.2°C) in a client with SCD requires investigation due to functional asplenia and high infection risk, but it is a less immediate threat than a compromised airway and breathing pattern. The "clear lungs" assessment is falsely reassuring. A client can be developing ACS with a normal initial chest exam, as the infiltrate and consolidation may lag behind the clinical signs of tachypnea and shallow breathing
[3]. The presence of clear lungs does not rule out the imminent onset of ACS, whereas the rapid, shallow breathing is a direct, real-time indicator of pulmonary decompensation.
The Critical Link Between VOC and ACS
The progression from VOC to ACS is a well-documented and dangerous clinical trajectory. A prospective study identified that ACS develops in a significant portion of adults admitted for VOC, and no single factor at presentation can perfectly predict it, making vigilant respiratory assessment the cornerstone of nursing care
[3]. The nurse’s role is to perform holistic, continuous monitoring to detect subtle changes . The finding of a respiratory rate of 32 with shallow breathing is not a subtle change; it is a loud clinical alarm. The immediate nursing intervention is to implement strategies to prevent or mitigate ACS, including aggressive incentive spirometry, optimal pain control to allow for deep breathing, and close monitoring for escalating oxygen requirements, all of which are core components of emergency management guidelines
