Understanding the Clinical Presentation
The patient is exhibiting a classic triad of neurological symptoms—severe headache, confusion, and blurred vision—in the setting of acute leukemia. The laboratory values reveal a white blood cell (WBC) count of
150,000/μL with
80% blasts, profound thrombocytopenia (platelets
15,000/μL), and anemia (hemoglobin
7.2 g/dL). This constellation points directly to a life-threatening hematologic emergency:
hyperleukocytosis with
leukostasis.
Pathophysiology of the Emergency
Hyperleukocytosis, most often defined as a WBC count greater than
100,000/μL, is associated with increased morbidity and early mortality primarily due to leukostasis [1,4]. Leukostasis occurs when a high burden of circulating blasts, which are larger and less deformable than mature leukocytes, increases blood viscosity. This leads to sludging of blasts in the microvasculature, particularly in the brain and lungs, causing vascular occlusion and tissue hypoxia. The patient's neurological symptoms are a direct consequence of this process in the cerebral vessels. While the patient has severe thrombocytopenia, the priority is not the risk of bleeding but the active, ongoing end-organ damage from vascular obstruction.
Prioritizing the Intervention
The nurse's priority intervention is to notify the physician immediately and prepare for emergency
leukapheresis (option 4). The rationale is grounded in the need for rapid cytoreduction to reverse the leukostasis syndrome. While prompt initiation of chemotherapy is paramount for treating the underlying leukemia, leukapheresis serves as a critical adjunctive strategy that can rapidly and safely reduce the white blood cell count, thereby decreasing blood viscosity and alleviating the acute symptoms of leukostasis [1,3].

Why not administer analgesics (Option 1)? This addresses a symptom, not the underlying pathology of microvascular occlusion. Masking the headache without treating the leukostasis could delay life-saving intervention and lead to irreversible neurological injury or death.

Why not prepare for platelet transfusion (Option 2)? Although the platelet count is critically low, prophylactic platelet transfusion is relatively contraindicated in leukostasis without active, life-threatening bleeding. Increasing platelet count can paradoxically worsen blood viscosity and exacerbate the vaso-occlusive crisis. The immediate threat is from hyperviscosity, not hemorrhage.

Why not initiate oxygen therapy as the priority (Option 3)? While the patient is anemic and may have concurrent pulmonary leukostasis causing hypoxia, supplemental oxygen does not resolve the underlying mechanical obstruction caused by leukemic blasts. It is a supportive measure, not a definitive treatment for the emergency.
Clinical Evidence and Procedural Efficacy
The use of leukapheresis in hyperleukocytic acute myeloid leukemia (AML) is a recognized intervention to rapidly lower the circulating blast burden
[3]. A retrospective analysis of 74 adult AML patients with WBC >
100 × 109/L demonstrated that leukapheresis is employed to manage life-threatening complications like the one presented in this scenario
[3]. Furthermore, a 20-year single-center study confirmed that for newly diagnosed AML patients presenting with hyperleukocytosis, leukocytapheresis is a standard immediate intervention aimed at preventing severe complications and early mortality
[4]. The procedure directly addresses the pathophysiological crisis, making it the nursing priority to facilitate this order by alerting the physician and preparing the necessary equipment and patient support.
