Understanding the Priority: Infection Risk in Bone Marrow Suppression
Chemotherapy for multiple myeloma targets rapidly dividing cells, a mechanism that effectively destroys malignant plasma cells but also profoundly suppresses the bone marrow. Analysis of the Correct Intervention
Monitoring

neutropenic precautions is the direct, evidence-based clinical management strategy to protect the client during the vulnerable nadir period

- Meticulous hand hygiene for all individuals entering the room.
- Restricting exposure to fresh flowers, standing water, and raw fruits or vegetables that harbor bacteria and fungi.
- Ensuring the client avoids rectal procedures, enemas, or suppositories that could breach the mucosal barrier.
- Immediately reporting any subtle signs of infection, such as a low-grade fever or mild sore throat.

Why Other Options Are Not the Priority

Option 3: Administer prescribed pain medications for bone pain relief. Bone pain is a significant comfort and quality-of-life issue in multiple myeloma. Opioid and non-opioid analgesics are essential for symptom management. However, pain control, while crucial, is not a life-sustaining intervention. A client can survive with managed pain but can die rapidly from an undetected infection during a neutropenic episode.

Option 4: Provide a high-protein diet to support tissue repair needs. Adequate nutrition supports overall healing and immune function. However, a high-protein diet is a supportive, long-term strategy. It does not provide immediate protection against the environmental pathogens that pose an existential threat to a client with an ANC near zero.
