Understanding the Priority: Fluid Resuscitation in Acute Pancreatitis
The client's presentation—fever, tachycardia, hypotension, and tachypnea—in the setting of a significantly elevated lipase and leukocytosis indicates a systemic inflammatory response. In acute pancreatitis (AP), activated pancreatic enzymes and inflammatory mediators cause massive vasodilation and increased capillary permeability, leading to a profound shift of fluid from the intravascular space into the interstitial "third space." This intravascular volume depletion is the primary driver of hypotension and tachycardia and, if uncorrected, can rapidly progress to hypovolemic shock and organ failure. The initial
24 to 72 hours are critical, as the management decisions made during this period directly influence clinical outcomes, including the development of pancreatic necrosis and organ failure
[2].
Therefore, the priority nursing intervention is to maintain NPO (nil per os) status and meticulously monitor fluid balance. Placing the client NPO provides pancreatic rest by eliminating the cephalic and gastric phases of pancreatic enzyme secretion, which is a cornerstone of initial management. However, the more time-sensitive component of this intervention is the assessment of fluid balance to guide aggressive intravenous fluid resuscitation. Current guidelines emphasize that early, goal-directed fluid therapy is the single most important non-supportive intervention in the first phase of AP [2,3]. The nurse's role is to monitor intake and output strictly, assess for signs of fluid overload or continued deficit (e.g., vital sign trends, skin turgor, mucous membranes, urine output), and titrate IV fluids as prescribed, typically with Lactated Ringer's solution, to restore intravascular volume and maintain organ perfusion
[2].
Analysis of Other Options
While the other interventions are important components of comprehensive care, they do not address the most immediate, life-threatening physiological derangement.

Administer prescribed analgesics (Option 1): Pain management is a critical and early intervention in AP to reduce metabolic stress and splinting, which can impair breathing
[2]. However, in this scenario, the client's vital signs point to a developing hypovolemic state. While pain medication should not be delayed unnecessarily, stabilizing the client's hemodynamic status through fluid resuscitation takes immediate precedence to prevent shock.

Encourage deep breathing exercises (Option 2): Respiratory complications, such as pleural effusions and atelectasis, are common in AP due to the proximity of the inflammatory process to the diaphragm and pain-induced shallow breathing. Encouraging deep breathing is a valuable preventive measure, but it is not the priority over correcting the life-threatening hypovolemia indicated by the client's blood pressure of
95/60 mmHg and heart rate of
110 bpm.

Monitor blood glucose levels (Option 3): The inflammatory destruction of pancreatic tissue can transiently impair endocrine function, specifically insulin production from beta cells, leading to hyperglycemia. Monitoring blood glucose is a necessary ongoing assessment, but it is a secondary concern compared to the immediate need for hemodynamic stabilization through fluid resuscitation and pancreatic rest.
The clinical picture of a client in the early phase of acute pancreatitis with signs of systemic inflammatory response syndrome (SIRS) and hypovolemia makes the maintenance of NPO status and rigorous monitoring of fluid balance the definitive priority nursing action [1,2,3].
