115 bpm is a compensatory tachycardia, and a respiratory rate of
26/min suggests the body is attempting to compensate for metabolic acidosis from hypoperfusion. The pallor and diaphoresis are clinical manifestations of the sympathetic nervous system's response to profound
hypovolemic shock. This combination of findings signals a life-threatening emergency where the immediate priority is not to stop the bleeding directly, but to preserve vital organ perfusion.
Prioritization and Rationale for the First Action
The correct first action is to
establish large-bore IV access and initiate fluid resuscitation. The physiological rationale is grounded in the principles of managing hypovolemic shock. The client's mean arterial pressure is critically low, leading to inadequate tissue oxygenation and a high risk of progression to irreversible shock, multi-organ failure, and cardiac arrest. Prepare the client for emergency endoscopy: Endoscopy is the definitive diagnostic and therapeutic procedure for variceal bleeding, allowing for band ligation or sclerotherapy.
