Clinical Reasoning and Prioritization
The most concerning finding requiring immediate intervention is
asterixis with
altered mental status. This combination represents a clinical manifestation of
hepatic encephalopathy (HE), a neuropsychiatric complication of liver disease that signals a significant decompensation event. As a nurse, recognizing that a change in neurological status takes precedence over chronic, stable dermatological or mucosal findings is critical for client safety.
Hepatic encephalopathy is not merely a symptom of advanced liver disease; it is a dangerous complication that carries a greater risk of death than any other major hepatic decompensation event
[1]. The underlying pathophysiology involves the gut-liver-brain axis, where the failing liver cannot effectively clear gut-derived neurotoxins, primarily
ammonia. This leads to a spectrum of neuropsychological abnormalities, from subtle cognitive deficits to the confusion and coma seen in this client
[1]. The presence of
asterixis, often described as a "liver flap," is a classic physical sign of metabolic encephalopathy and indicates a progressive and severe decline in cerebral function. When coupled with a reported increase in confusion over the past
24 hours, it points to an acute worsening of HE, which can rapidly progress to cerebral edema, coma, and death without prompt intervention.
The other options, while consistent with the diagnosis of end-stage cirrhosis, do not represent an immediate threat to the client's airway, breathing, or neurological function.
Jaundice and
scleral icterus (Option 1) are chronic findings resulting from the liver's inability to conjugate and excrete bilirubin.
Spider angiomata (Option 3),
palmar erythema, and
clubbing (Option 4) are vascular and dermatologic changes linked to altered estrogen metabolism and chronic hypoxemia, respectively. These are expected, stable physical assessment findings in a client with long-standing cirrhosis and do not signal an acute, life-threatening change in status. The nurse’s priority is to identify and escalate the finding that indicates an acute decompensation, which is the neurological deterioration. Furthermore, the gut microbiome and systemic inflammation are now recognized as key drivers of HE
[1], and a worsening mental status in a client with increasing abdominal distension (ascites) must also raise suspicion for an underlying precipitating infection, such as
spontaneous bacterial peritonitis (SBP), which is a life-threatening complication in cirrhotic patients with refractory ascites .
