Understanding the Priority in the Immediate Postoperative Period
Following a parathyroidectomy, the most critical physiological change to anticipate is a sudden drop in parathyroid hormone (PTH) levels. In a client with hyperparathyroidism, the overactive parathyroid glands have been maintaining a high bone turnover state. When the source of excess PTH is surgically removed, PTH levels plummet. PTH normally acts on bones and kidneys to increase serum calcium; without it, bones rapidly take up calcium from the blood, and the kidneys excrete less calcium but the net effect is a profound decrease in serum calcium. This condition is known as
hungry bone syndrome (HBS), a severe, rapid, and sometimes fatal hypocalcemia that can occur after parathyroidectomy
[1]. The risk is significant, with studies reporting a
20-70% incidence of HBS following surgery for secondary hyperparathyroidism
[2], and hypocalcemia remains the most common complication even in primary hyperparathyroidism cases
[3].
Why Hypocalcemia Assessment Takes Priority
The immediate postoperative period, specifically the first few hours, is when the serum calcium level can fall most dramatically. The clinical manifestations of acute hypocalcemia are life-threatening and require immediate detection. These include neuromuscular irritability, which can progress from perioral numbness and tingling in the fingers (paresthesias) to muscle cramps, tetany, laryngospasm, and seizures. A positive
Chvostek sign (facial muscle twitching upon tapping the facial nerve) or
Trousseau sign (carpal spasm induced by a blood pressure cuff) are classic indicators. Cardiac effects such as a prolonged QT interval on the ECG can degenerate into dangerous arrhythmias. Because these complications can develop swiftly and become fatal, monitoring for hypocalcemia is the nurse's highest priority, aligning with the airway, breathing, and circulation (ABC) framework and safety principles.
Analyzing the Other Options

Option 1: Monitor for signs of infection at the surgical site. While surgical site infection is a valid postoperative concern, it is not the priority in the first few hours. Signs of infection, such as redness, purulent drainage, and fever, typically take 24 to 72 hours or more to manifest. This intervention is important but is secondary to a complication that can cause immediate life-threatening events.

Option 3: Encourage early ambulation to prevent complications. Early ambulation is a standard postoperative intervention to prevent complications like atelectasis, pneumonia, and venous thromboembolism. However, in a client at high risk for acute hypocalcemia, ambulation could be dangerous. If the client develops muscle cramps, tetany, or a seizure while walking, they are at high risk for a fall and injury. The client's calcium status and neuromuscular stability must be assessed and managed before ambulation is safely encouraged.

Option 4: Administer pain medication as prescribed. Pain management is an essential nursing responsibility. However, administering an analgesic does not address a potentially fatal electrolyte imbalance. Furthermore, a client's altered mental status or neuromuscular irritability from severe hypocalcemia should not be mistaken for or masked by pain medication. Assessment of a more critical physiological threat must precede the administration of any medication.
Clinical Application and Risk Stratification
The nurse's assessment should be systematic and focused. It begins with monitoring vital signs, paying close attention to the respiratory rate and pattern, as laryngospasm can compromise the airway. Continuous cardiac monitoring is essential to observe for a prolonged QT interval. The nurse must check serial serum calcium levels as ordered and have intravenous calcium gluconate or calcium chloride readily available for emergency replacement. Research has identified that both symptomatic and asymptomatic patients with primary hyperparathyroidism are at risk for postoperative hypocalcemia, necessitating individualized calcium management protocols for all patients
