NCLEX-RN Clinical Application: Initiating ACE Inhibitor Therapy
When starting a patient on an angiotensin-converting enzyme (ACE) inhibitor such as
lisinopril, the most critical nursing intervention is to
monitor for orthostatic hypotension and educate about position changes. This priority stems directly from the drug's mechanism of action and the predictable physiological response during the initial treatment phase.
ACE inhibitors block the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor. This leads to arteriolar and venous dilation, reducing systemic vascular resistance and blood pressure. The first dose can cause a pronounced drop in blood pressure, a phenomenon known as
first-dose hypotension, which is particularly dangerous when a patient moves from a supine or sitting position to standing. The resulting
orthostatic hypotension significantly increases the risk of syncope, falls, and injury. As highlighted in the current hypertension guidelines, a pragmatic, primary care-focused approach to managing hypertension necessitates anticipating and mitigating such adverse effects to ensure patient safety and medication adherence [1,2].
The rationale for this intervention is further reinforced by evidence from heart failure management, where ACE inhibitors and angiotensin receptor-neprilysin inhibitors (ARNIs) are cornerstones of guideline-directed medical therapy (GDMT). A major barrier to initiating and up-titrating these life-saving medications is the occurrence of hypotension. Clinical reports indicate that hypotension is a common reason for delayed initiation, dose reduction, or discontinuation of therapy, which directly undermines treatment goals
[3]. By proactively monitoring for this effect, the nurse plays a pivotal role in enabling the patient to safely reach and maintain their target dose.
The other options are incorrect and potentially harmful. Encouraging increased sodium intake directly contradicts the dietary modifications essential for hypertension management, which aim to lower blood pressure. Administering lisinopril with food is not required for absorption and does not mitigate the risk of first-dose hypotension. Most critically, instructing a patient to take the medication only when blood pressure is elevated promotes dangerous fluctuations in blood pressure and negates the protective, long-term cardiovascular benefits of consistent therapy. The goal of treatment is to maintain a controlled, steady-state blood pressure, not to chase intermittent spikes. The consistent underutilization of GDMT observed in various settings underscores the importance of nurses not only administering the medication but also actively managing the barriers, like hypotension, that lead to treatment gaps .
The priority nursing action is to obtain a baseline blood pressure and heart rate, administer the dose, and then monitor the patient's response, particularly after the first dose. Specific education must include instructing the patient to rise slowly from a lying to a sitting position, pause for a moment, and then stand up gradually. They should be taught to recognize symptoms of hypotension such as dizziness or lightheadedness and to sit or lie down immediately if these occur. This anticipatory guidance is fundamental to safe initiation of ACE inhibitor therapy.
