A sudden onset of severe abdominal or back pain with hypotension is a surgical emergency, most commonly indicating a ruptured abdominal aortic aneurysm (AAA). This represents active hemorrhagic shock from blood loss into the retroperitoneal space.
The classic clinical triad is hypotension, severe acute pain, and a pulsatile abdominal mass. The presence of hypotension signifies hemodynamic instability and a high risk of imminent cardiovascular collapse. Immediate notification of the HCP and surgical team is the highest priority.
Do not delay notification for imaging if the patient is hemodynamically unstable. Palpation of the mass should be gentle to avoid dislodging clot. Prepare for massive transfusion protocol and immediate surgical intervention.
