Understanding the Priority Setting Framework
In the NCLEX-RN examination, when a question asks for the "highest priority" nursing action, you must apply a clinical decision-making framework. For a patient who has just received
thrombolytic therapy, the most immediate and life-threatening risk is
hemorrhage, particularly
intracranial hemorrhage (ICH). This makes patient safety and the detection of complications the absolute priority over other important, but less time-sensitive, interventions. The nursing process dictates that assessment comes first, and in this scenario, the assessment with the most critical safety implications is monitoring for bleeding.
Why Option 1 is the Highest Priority
Administering a thrombolytic agent like tenecteplase works by converting plasminogen to plasmin, which actively breaks down the fibrin mesh within clots. This systemic fibrinolytic state is not selective; it dissolves pathological clots causing an acute coronary syndrome but also impairs the body's ability to form stable clots at any site of vascular injury. The most devastating complication of this therapy is
intracranial hemorrhage, a finding supported by the meta-analysis on tenecteplase for ischemic stroke, which inherently balances reperfusion efficacy against the risk of bleeding
[1]. Therefore, the highest priority nursing action is vigilant surveillance for its earliest signs. A change in
neurological status is the cardinal indicator of an evolving ICH. By assessing neurological status every
15 minutes, the nurse is performing focused, high-frequency assessments to detect subtle changes such as an altered level of consciousness, new-onset headache, unilateral weakness, or pupillary changes. Concurrently, monitoring all other body systems for overt and occult signs of bleeding—such as from IV sites, gums, urine, stool, and skin—is a core component of this safety assessment. This action directly addresses the greatest potential for patient harm in the immediate post-thrombolytic phase.
Why the Other Options are Lower Priority

Option 2: Encourage early ambulation to prevent deep vein thrombosis. This action is contraindicated in the acute phase of treatment for unstable angina and following thrombolytic therapy. The patient is at high risk for cardiac dysrhythmia and re-occlusion, and the invasive lines (e.g., arterial sheaths) present a serious bleeding risk if dislodged. Bed rest is maintained to decrease myocardial oxygen demand and protect vascular access sites.

Option 3: Administer sublingual nitroglycerin for chest pain relief. While managing chest pain is an important goal in acute coronary syndrome, it is not the highest priority immediately after thrombolysis. The primary goal of the lytic therapy is to reperfuse the myocardium, which should resolve the ischemic chest pain. If pain recurs, it may indicate re-occlusion, but the nurse's first action is to assess the patient (including a 12-lead ECG and vital signs) and then administer a prescribed medication. The assessment for a life-threatening complication (bleeding) takes precedence over administering a medication for a symptom.

Option 4: Obtain a 12-lead ECG every 4 hours to monitor cardiac rhythm. Monitoring for reperfusion arrhythmias and ST-segment resolution is a critical part of post-thrombolytic care. However, an ECG every
4 hours is a routine monitoring interval. The risk of a catastrophic bleed is highest in the first few hours after therapy, requiring neurological and bleeding assessments at a much more frequent interval (every
15 minutes). The assessment that detects the most immediately life-threatening complication is the highest priority.
The decision-making process of a surrogate deciding on thrombolysis is heavily influenced by the perceived risk of this exact complication . The nurse's role is to operationalize the safety monitoring that mitigates this risk, making the detection of bleeding the unequivocal priority. This principle applies across the spectrum of thrombolytic use, from acute coronary syndromes to ischemic stroke, where the therapeutic window is continuously being explored to balance benefit against the constant hazard of hemorrhage [1,4]. Even in other critical scenarios like intra-arrest thrombolysis, the risk-benefit analysis centers on this same safety profile .
